The "6-Month Pregnant" Bloat: Why Your Middle Stomach Swells

Ever wake up with a flat stomach, only to look six months pregnant by mid afternoon? You are not imagining it, and it is not just fat or water weight. When bloating concentrates right around your belly button and upper abdomen, the root cause usually lives in your middle digestive tract: your small intestine, gallbladder, and pancreas. Understanding how this region works is often the missing piece to uncovering why your body is reacting this way.
How Your Middle Gut Works
After food leaves your stomach, it enters the first part of your small intestine (the duodenum). Here, stomach acid is neutralized by bicarbonate from your pancreas, while pancreatic enzymes and gallbladder bile break down your food (Bartel, 2025).
As food moves deeper into the next sections (the jejunum and ileum), millions of tiny fingerlike folds absorb your nutrients and water, while muscle contractions churn everything to keep it moving toward your colon (Bartel, 2025).
Location Matters: Where Microbes Belong
Your gut is not meant to have the same amount of bacteria everywhere. In fact, bacteria levels change dramatically depending on where you look (Wlodarska et al., 2015, as cited in Wang et al., 2024):
Stomach & Upper Small Intestine: Very low—only 10 to 1,000 bacterial cells per gram of fluid (O’Hara & Shanahan, 2006, as cited in Wang et al., 2024).
Middle Small Intestine: Growing steadily—10,000 to 100 million cells per gram (O’Hara & Shanahan, 2006, as cited in Wang et al., 2024).
The Colon (Large Intestine): Your main microbe hub—teeming with 100 billion to 1 trillion cells per gram (O’Hara & Shanahan, 2006, as cited in Wang et al., 2024).
When large amounts of bacteria move up into the small intestine—where populations are supposed to stay low—they ferment food early, causing intense visible swelling and "pregnant style" bloating.
Your Gut’s Internal Housekeeper
While you eat, your gut squeezes and mixes food. But when you are not eating, a completely different system takes over: the Migrating Motor Complex (MMC) (Jiang et al., 2017). Think of the MMC as your gut’s internal housekeeping crew. Happening every 2 to 4 hours while fasting or sleeping, it runs through four distinct stages (Foreman et al., 2024; Jiang et al., 2017):
Phase I: Quiet rest.
Phase II: Gentle warming up contractions.
Phase III (The Housekeeping Wave): A short, powerful burst of contractions that sweeps leftover food, debris, and stray bacteria down into the colon (Foreman et al., 2024).
Phase IV: A quick cool down back to rest.
This clearing wave is triggered by a hormone called motilin along with serotonin (Takahashi, 2013). The moment you take a bite of food, motilin turns off and the housekeeping wave stops so your body can focus on digesting (Takahashi, 2013). Snacking constantly keeps your housekeeper off duty!
Why Does Gut Movement Slow Down?
When your gut's natural wave movements slow down, food sits too long, creating a breeding ground for overgrowth (Cleveland Clinic, 2022).
What Disrupts Your Gut Movement?
Medications Opioid pain relievers, nerve blocking meds, calcium channel blockers, and certain antidepressants (Cleveland Clinic, 2022).
Metabolic Health Hypothyroidism (slow metabolism) and diabetes related nerve damage (Cleveland Clinic, 2022).
Nervous System Conditions affecting gut nerve signaling (Cleveland Clinic, 2022).
Surgery & Infection Scar tissue (adhesions), previous abdominal surgeries, or recent gut infections (Cleveland Clinic, 2022).
Electrolytes Low potassium or calcium, which muscles need to contract properly (Cleveland Clinic, 2022).
